Health insurance​ can refer to a variety of insurance policies​ depending on what t​hey cover, how they ar​e purchased and paid for, and the rules that govern the health plan.

This guide provides general guidelines on types of health care insurance coverage​, health plan designs, health plan options including those available through the Affordable Care Act, important terms and definitions, and the grievance​ and independent review​ process. ​​​
