​​To submit certain reports, you must have a MyWisconsin ID account.

Reports Requiring a MyWisconsin ID

You will need a MyWisconsin ID to submit the following reports:

• Health Insurance Enrollment Reporting

• Grievance Reporting

• Long-Term Care Reporting

• Medicare Supplement Reporting

• Managed Care Plan Certifications

• Certified Application Counselor Entities/Individuals

• Cybersecurity Event Notification

Set Up a MyWisconsin ID

If you do not already have a MyWisconsin ID, you will need to create one.

MyWisconsin ID requires multi-factor authentication (MFA) which will make it difficult to use a shared email account with this user ID.

After your MyWisconsin ID is created, return to this page and click the link for the report you want to submit. That link will provide you with information specific to the report.

Reports That Do Not Require a MyWisconsin ID

You can submit the following reports without a MyWisconsin ID:

• Market Conduct Annual Statement (MCAS)

• Multiple Employer Trust Filing
