Madison, Wis. — The Office of the Commissioner of Insurance (OCI) is responsible for administering and enforcing the insurance laws of Wisconsin. This includes monitoring the financial and marketing practices of individuals and companies. Each month, OCI publishes the administrative actions it has taken against these entities. The actions are issued by OCI when licensing applicants, or licensed individuals or companies have violated Wisconsin insurance statutes or regulations. In some cases, the individual or company denied the stated allegations but consented to the action taken. Any forfeitures paid are deposited in Wisconsin's Common School Fund administered by the Board of Commissioners of Public Lands. Earnings from the Fund are distributed to all public K-12 schools in the state and are used by school libraries to purchase materials such as books, newspapers, and computers.

Allegations and Actions Against Agents
Elisabeth M. Dugar, 1150 Suburban St., Cedar Hill, TX 75104, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Andrew Ebbole, 1241 S. Webster Ave., Green Bay, WI 54301, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Daaniyl Jackson, 2301 Bunche Park School Dr., Opa-Locka, FL 33054, had his insurance license permanently revoked. This action was taken based on allegations of misrepresentation, unsuitable sales, non-appointment, and failure to respond.
Donald McPhail, 900 DC Ct. SE, Pine Island, MN 55963, agreed to the permanent revocation of his insurance license. This action was taken based on allegations of misrepresentation and unsuitability.
Jennifer A. Stedman, 2753 Mayport Rd., Apt. 2536, Jacksonville, FL 32233, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Guadalupe Villarreal, 512 Lovett Rd., San Benito, TX 78586, had her insurance license revoked. This action was taken based on allegations of failing to disclose a criminal conviction, having a criminal conviction which may be substantially related to the circumstances of holding an insurance license, and failing to participate as ordered in an administrative hearing.
Ruth Whitehead, 508 14th Ave., Baraboo, WI 53913, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Allegations and Actions Against Companies
Andy Tom Agency Inc., 3046 W. Irving Park, Chicago, IL 60618, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.
Transamerica Life Insurance Company, 6400 C Street SW, Cedar Rapids, IA 52499, agreed to offer to rescind and refund consumers for two life insurance policies. This action was taken based on allegations against its agent of misrepresenting the terms of the policies.
Allegations and Actions Against Other Entities
Strategic Limited Partners, 1452 Hughes Rd., Ste. 200, Grapevine, TX 76051, and OCI agreed to a compromise stipulation and order. This action was based on allegations of operating without a license and misrepresentation.
