Madison, Wis. — The Office of the Commissioner of Insurance (OCI) is responsible for administering and enforcing the insurance laws of Wisconsin. This includes monitoring the financial and marketing practices of individuals and companies. Each month, OCI publishes the administrative actions it has taken against these entities. The actions are issued by OCI when licensing applicants, or licensed individuals or companies have violated Wisconsin insurance statutes or regulations. In some cases, the individual or company denied the stated allegations but consented to the action taken. OCI strives to ensure fair and honest business practices to protect Wisconsin insurance consumers through this type of transparency. Any forfeitures paid are deposited in Wisconsin's Common School Fund administered by the Board of Commissioners of Public Lands. Earnings from the Fund are distributed to all public K-12 schools in the state and are used by school libraries to purchase materials such as books, newspapers, and computers.

Allegations and Actions Against Agents

Derek M. Bennett, 5728 Seton Dr., Margate, FL 33063, had his application for an insurance license denied.  This action was taken based on allegations of failing to disclose administrative actions taken by other states, and failing to disclose an appointment termination for cause.

Guesline M. Calixte, 6028 Woodlands Blvd., Tamarac, FL 33319-2525, agreed to the permanent revocation of their Wisconsin insurance license. This action was taken based on allegations of enrolling customers in insurance plans without their knowledge or consent.

Larry M. Contreraz, 1050 E. Perrin Ave., Ste. 107, Fresno, CA 93720, had his application for an insurance license denied.  This action was taken based on allegations of a previous Wisconsin administrative action involving misrepresentation of an insurance product.

Matthew Deets, 209 Robb St., Schofield, WI 54476, had his application for an insurance license denied.  This action was taken based on allegations of failing to comply with a court order imposing a child support obligation and criminal convictions substantially related to the circumstances of holding an insurance license.

Edward A. Duke, 14607 N. 63rd Pl., Scottsdale, AZ 85254, had his application for an insurance license denied.  This action was taken based on allegations of owing delinquent Arizona taxes.

Tavisha Grant, 625 W. Southern Ave., Ste. E, Mesa, AZ 85210, agreed to surrender her public adjuster registration.  This action was taken based on allegations of failing to maintain a public adjuster bond.​

Andre Hopson, 7459 Richmond Rd., Memphis, TN 38125, had his insurance license revoked.  This action was taken based on allegations of owing delinquent Wisconsin taxes.

Amy J. Lichter, 2820 14th Ln., Kenosha, WI 53140, had her application for an insurance license denied.  This action was taken based on allegations of having criminal convictions that may be substantially related to the circumstances of holding an insurance license.

Nicolas A. Liogas, 6838 Revere Ct., Gurnee, IL 60031, had his application for an insurance license denied.  This action was taken based on allegations of failing to disclose administrative actions on a license application.

Geoffrey Privette, 380 Jessicas Way, Johnson City, TN 37615, agreed to the permanent revocation of their Wisconsin insurance license. This action was taken based on allegations of enrolling customers in insurance plans without their knowledge or consent.

Roy Strate, 9510 W. Sahara Ave., Unit 125, Las Vegas, NV 89117, agreed to the permanent revocation of their Wisconsin insurance license. This action was taken based on allegations of enrolling a customer in an insurance plan without their knowledge or consent.

Candyce Tritt, W9104 County Rd. A, Crivitz, WI 54114, had her insurance license revoked.  This action was taken based on allegations of owing delinquent Wisconsin taxes.

Kirsten Vannest, 218 Wilson Way, Marshfield, MO 65706-2052, had her application for an insurance license denied. This action was taken based on allegations of having criminal convictions which may be substantially related to the circumstances of holding an insurance license.

Hailee N. Werkmeister, 642 2nd Blvd. PVT., Hancock, WI 54943, had her insurance license revoked.  This action was taken based on allegations of owing delinquent Wisconsin taxes.

Wardean Whitt, 15483 N. Jackson St., Durant, MS 39063-4003, had her application for an insurance license denied. This action was taken based on allegations of having regulatory action taken with regard to an insurance license held in another state, misrepresenting the terms of an insurance contract or application for insurance, committing an insurance unfair trade practice, and forging another's name on an application for insurance or other document related to an insurance transaction.

Allegations and Actions Against Companies

Alan Sword LLC, 13960 Highway 31 N., Memphis, IN 47143, agreed to surrender their intermediary firm license. This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.

Broad Street Brokerage Insurance Agency LLC, PO Box 182155, Columbus, OH 43272-7485, agreed to pay a forfeiture of $500.00 and surrendered their intermediary firm license.  This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.

Gemrock Agency LLC, 5702 Ave. N., Brooklyn, NY 11234, was ordered to pay a forfeiture of $1,000.00.  This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.

Insureyourpeople LLC, 40 E. Rio Salado Pkwy., #900, Tempe, AZ 85281, was ordered to pay a forfeiture of $1,000.00.  This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.

Intalere Insurance Services LLC, 290 E. John Carpenter Fwy., Irving, TX 75062, was ordered to pay a forfeiture of $1,000.00.  This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.

Iron Health Group LLC, 2095 Highway 211 NW, Ste. 2F, Braselton, GA 305172, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.

OTR Risk Management LLC, PO Box 337, Waxhaw, NC 28173, was ordered to pay a forfeiture of $500.00.  This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.

Premium Choice Insurance Services, 16501 Ventura Blvd., Ste. 400, Encino, CA 91436, was ordered to pay a forfeiture of $1,500.00. This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.

Secur-All Agency Inc., One Dupont St., Plainview, NY 11803, was ordered to pay a forfeiture of $1,500.00.  This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.

Susan Moore & Associates Inc., 1130 W. Lake Cook Rd., Ste. 195, Buffalo Grove, IL 60089, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.

Wilson Mathew Nechikat Insurance Agency Inc., 9511 Mainline Dr., Elk Grove, CA 95624, was ordered to pay a forfeiture of $1,500.00. This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.​
