Madison, Wis. — The Office of the Commissioner of Insurance (OCI) is responsible for administering and enforcing the insurance laws of Wisconsin. This includes monitoring the financial and marketing practices of individuals and companies. Each month, OCI publishes the administrative actions it has taken against these entities. The actions are issued by OCI when licensing applicants, or licensed individuals or companies have violated Wisconsin insurance statutes or regulations. In some cases, the individual or company denied the stated allegations but consented to the action taken. OCI strives to ensure fair and honest business practices to protect Wisconsin insurance consumers through this type of transparency. Any forfeitures paid are deposited in Wisconsin's Common School Fund administered by the Board of Commissioners of Public Lands. Earnings from the Fund are distributed to all public K-12 schools in the state and are used by school libraries to purchase materials such as books, newspapers, and computers.

Allegations and Actions Against Agents

Darlene Mosbaugh, 75443 Schmenski Rd., Glidden, WI 54527, had her insurance license suspended for 60 days and was ordered to pay a forfeiture of $1,000.00. These actions were taken based on allegations of failing to promptly respond to OCI information requests, violating the terms of a previous Stipulation and Order, and demonstrating a lack of trustworthiness and competence.

Vijaya Bhaskar R. Puli, 549 Burnt Sienna Dr., Middleton, WI 53562, had his insurance license summarily suspended and agreed to the revocation of his insurance license. These actions were taken based on allegations of failing to have a valid work visa for employment in the insurance field and submitting duplicate licensing applications containing incomplete and inaccurate information.

Michael Shillin, 19062 62nd Ave., Chippewa Falls, WI 54729, agreed to pay a forfeiture of $5,000.00 and agreed to permanently surrender his insurance license. These actions were taken based on allegations of falsifying insurance records.

Blaine Sprague, E5455 Hannu Rd., Ironwood, MI 49938, had his insurance license summarily suspended and was ordered to cease and desist doing the business of insurance. These actions were taken based on allegations of submitting health insurance applications without consumer knowledge or authorization.

Addis A. Zewdie, 4820 N 90th St., Milwaukee, WI 53225, had her application for an insurance license denied. This action was taken based on allegations of failing to disclose administrative actions taken by the states of Wisconsin, Idaho, South Carolina, and Washington on a licensing application.

Allegations and Actions Against Companies​

CSA Fraternal Life, 2050 Finley Rd., Ste. 70, Lombard, IL 60148, was ordered to pay a forfeiture of $6,750.00, was ordered to remove certain language from its application materials, and was ordered to develop a corrective screening system. These actions were taken based on allegations of failing to ensure suitability in the sales of annuity products.

KSKJ Life, American Slovenian Catholic Union, 2439 Glenwood Ave., Joliet, IL 60435, was ordered to pay a forfeiture of $12,750.00, was ordered to remove certain language from its application materials, and was ordered to develop a corrective screening system. These actions were taken based on allegations of failing to ensure suitability in the sales of annuity products.​
