Madison, Wis. — The Office of the Commissioner of Insurance (OCI) is responsible for administering and enforcing the insurance laws of Wisconsin. This includes monitoring the financial and marketing practices of individuals and companies. Each month, OCI publishes the administrative actions it has taken against these entities. The actions are issued by OCI when licensing applicants, or licensed individuals or companies have violated Wisconsin insurance statutes or regulations. In some cases, the individual or company denied the stated allegations but consented to the action taken. OCI strives to ensure fair and honest business practices to protect Wisconsin insurance consumers through this type of transparency. Copies of the administrative orders may be viewed online at

Any forfeitures paid are deposited in Wisconsin's Common School Fund administered by the Board of Commissioners of Public Lands. Earnings from the Fund are distributed to all public K-12 schools in the state and are used by school libraries to purchase materials such as books, newspapers, and computers.

Allegations and Actions Against Agents
Timothy S. Bratley, 2900 Junction Rd., Ashland, WI 54806, had his insurance license permanently revoked and was ordered to pay a forfeiture of $37,375.00. These actions were taken based on allegations of engaging in unfair trade practices and making misrepresentations on insurance applications. Floyd N. Johnson, 5237 E County Rd. J, Clinton, WI 53525, was ordered to pay a licensing and returned item fee. This action was taken based on allegations of stopping payment on a licensing application fee.
Jace T. McDonald, 383 E Montello St., Montello, WI 53949, had his application to reinstate his insurance license denied. This action was taken based on allegations of failing to disclose a pending lawsuit, administrative actions taken by DFI and OCI, and employment history on a licensing application; selling life settlement products without a license; committing financial abuse; and failing to provide documentation required to complete a licensing application.
Diana R. Oyola, 2010 Porter Rd., Apt 1, Plover, WI 54467, was ordered to pay a licensing and returned item fee. This action was taken based on allegations of stopping payment on a licensing application fee.
Thania Romero, 112 S Wisconsin St., Apt. 106, Elkhorn, WI 53121, was ordered to pay a licensing and returned item fee. This action was taken based on allegations of stopping payment on a licensing application fee.
Jose L. Solorzano, 3800 S 52nd St., Milwaukee, WI 53220, had his application for a federal crime waiver denied. This action was taken based on having a criminal conviction that prohibits engaging in the business of insurance and failing to make restitution as ordered by the court.
Yeng Vang, 715 S 15th St., Sheboygan, WI 53081, had his application for an insurance license denied. This action was taken based on allegations of having a criminal conviction that may be substantially related to insurance marketing type conduct, failing to apply for and obtain a federal crime waiver, and failing to respond promptly to inquiries from OCI.
Jory J. Webster, 283 Ledgewood Dr., Fond du Lac, WI 54937, had his application for an insurance license denied. This action was taken based on allegations of having a criminal conviction that may be substantially related to insurance marketing type conduct, failing to disclose criminal convictions on a licensing application, and failing to apply for and obtain a federal crime waiver.
Allegations and Actions Against Companies
Chelsea Morgan Securities, Inc., 242 Main St., Staten Island, NY 10307, agreed to the issuance of a conditional insurance license having certain employee licensing and supervision requirements. This action was taken based on allegations of having a history of administrative actions and failing to fully disclose the administrative actions on a licensing application.
Employers Mutual Casualty Company, 717 Mulberry St., Des Moines, IA 50309, agreed to take corrective action, including employee training, to ensure compliance with Wisconsin's insurance laws related to dividend filing requirements. This action was taken based on allegations of issuing workers compensation policies with unfiled dividends.
