Madison, Wis. — The Office of the Commissioner of Insurance (OCI) is responsible for administering and enforcing the insurance laws of Wisconsin. This includes monitoring the financial and marketing practices of individuals and companies. Each month, OCI publishes the administrative actions it has taken against these entities. The actions are issued by OCI when licensing applicants, or licensed individuals or companies have violated Wisconsin insurance statutes or regulations. In some cases, the individual or company denied the stated allegations but consented to the action taken. OCI strives to ensure fair and honest business practices to protect Wisconsin insurance consumers through this type of transparency. Any forfeitures paid are deposited in Wisconsin's Common School Fund administered by the Board of Commissioners of Public Lands. Earnings from the Fund are distributed to all public K-12 schools in the state and are used by school libraries to purchase materials such as books, newspapers, and computers.

Allegations and Actions Against Agents

Timothy W. Baldry, 1350 S Ellsworth Rd., Apt. 1125, Mesa, AZ 85209, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to timely disclose to OCI administrative actions taken by the states of Indiana, North Dakota, and Arizona.

Samuel E. Belcher, 104 Shalom Dr., Simpsonville, SC 29681, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to timely disclose to OCI administrative actions taken by the states of North Carolina, Delaware, South Dakota, and Virginia.

Kieara Copeland, 1505 Island Ford Rd., Apt. 4, Madisonville, KY 42431, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to timely disclose to OCI administrative actions taken by the states of South Dakota, Louisiana, and California.

Jessica Delisca, 2808 Crescent Pl., Miramar, FL 33025, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to timely disclose to OCI administrative actions taken by the states of Indiana, New York, and Louisiana.

Fred Felder, 31149 Edendale Dr., Wesley Chapel, FL 33543, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to timely disclose to OCI administrative actions taken by the states of Louisiana, Virginia, South Dakota, and Washington. 

Ira P. Green, PO Box 259015, Plano, TX 75025, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to timely disclose to OCI administrative actions taken by the states of Louisiana, California, and Washington.

William M. Hall, 445 State St., Fremont, MI 49413, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to timely disclose to OCI administrative actions taken by the states of Louisiana, Indiana, and California. 

Marcus J. Holley, 6553 S Stevens Hollow Dr., Chesterfield, VA 23832, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to timely disclose to OCI administrative actions taken by the states of Louisiana, South Dakota, and California.

Jeffery Lewis, 617 Hazy Dawn Dr., Aberdeen, MD 21001, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to timely disclose to OCI administrative actions taken by the states of Louisiana, Ohio, and Pennsylvania.

Johnathan Lund, 5202 W Douglas Corrigan Way, Salt Lake City, UT 84116, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to timely disclose to OCI administrative actions taken by the states of Florida, Indiana, and California.

William J. Mandeline, 9604 Capendon Ave., Apt. 106, Palm Beach Gardens, FL 33418, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to timely disclose to OCI administrative actions taken by the states of Louisiana, Washington, and Arizona.

Zahirah L. Martinez, 6101 Phillip Ln., Lithonia, GA 30058, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to timely disclose to OCI administrative actions taken by the states of Indiana, Delaware, and Washington.

Paul McCaskill, 1023 Ridgeview Cir., Carrollton, TX 75007, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to timely disclose to OCI administrative actions taken by the states of Louisiana, Nebraska, Indiana, and Washington.

Shaheed Payne, 5806 N 12th St., Philadelphia, PA 19141, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to timely disclose to OCI administrative actions taken by the states of Washington, Illinois, North Carolina, and South Dakota.

Vijaya Bhaskar Reddy Puli, 549 Burnt Sienna Dr, Middleton, WI 53562, had his application for an insurance license denied. This action was taken based on failing to provide employment authorization to work in the insurance industry in the United States.

Alfred C. Shellman, 260 Jimmy Ann Dr., Daytona Beach, FL 32114, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to timely disclose to OCI administrative actions taken by the states of North Dakota, South Dakota, and Louisiana.

Amanda Simon, c/o CUNA Mutual Group, 2000 Heritage Way, Waverly, IA 50677, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to timely disclose to OCI administrative actions taken by the states of California, Wyoming, Louisiana, and Washington.

Alexander R. Walton, 10709 Tesch Ln., Apt 46, Rothschild, WI 54474, had his application for an insurance license denied. This action was taken based on allegations of having criminal convictions that may be substantially related to in​surance marketing type conduct and having pending criminal charges.

Alan W. Watkins, 10900 Bustleton Ave., Apt. B49, Philadelphia, PA 19116, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to timely disclose to OCI administrative actions taken by the states of Maryland, Delaware, and South Dakota.

Paul B. Wells, 241 Zion Dr, Las Vegas, NV 89107, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to timely disclose to OCI administrative actions taken by the states of Louisiana, Washington, and South Dakota.​
