Madison, Wis. — The Office of the Commissioner of Insurance (OCI) is responsible for administering and enforcing the insurance laws of Wisconsin. This includes monitoring the financial and marketing practices of individuals and companies. Each month, OCI publishes the administrative actions it has taken against these entities. The actions are issued by OCI when licensing applicants, or licensed individuals or companies have violated Wisconsin insurance statutes or regulations. In some cases, the individual or company denied the stated allegations but consented to the action taken. OCI strives to ensure fair and honest business practices to protect Wisconsin insurance consumers through this type of transparency. Copies of the administrative orders may be viewed online at

Any forfeitures paid are deposited in Wisconsin's Common School Fund administered by the Board of Commissioners of Public Lands. Earnings from the Fund are distributed to all public K-12 schools in the state and are used by school libraries to purchase materials such as books, newspapers, and computers.

Mark Banks II, 7643 W. Clovernook St., Milwaukee, WI 53223, had his application for an insurance license denied. This action was taken based on allegations of owing delinquent child support, providing untrue information on a licensing application, and failing to timely respond to inquiries from OCI.
Adrian R. Bernal, 7302 Chenal Pt., San Antonio, TX 78240, had his application for an insurance license denied. This action was taken based on allegations of failing to disclose criminal convictions and providing misleading and inaccurate information on a licensing application.
Sharon Davis, c/o 7349 IBM Dr., Ste. 103, Charlotte, NC 28262, had her application for an insurance license denied. This action was taken based on allegations of having a criminal conviction that may be substantially related to insurance marketing type conduct and providing misleading information on a licensing application.
Aaron F. Dixon, 3625 London Rd., Eau Claire, WI 54701, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Glenn R. Paterson, 211 Elderberry Dr., Goleta, CA 93117, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Carl Reed IV, 32012 N Marginal Rd., Apt. 272, Willowick, OH 44095, was ordered to pay a forfeiture of $500.00 and was ordered to provide information to OCI. This action was taken based on allegations of failing to timely respond to OCI inquiries.
Thomas A. Roule, Sr., 9 Stirrup Cup Ct., St. Charles, IL 60174, had his application for an insurance license denied. This action was taken based on allegations of failing to disclose an administrative action taken by the State of Illinois and failing to provide documentation required to complete a licensing application.
Barry M. Rumpel, W833 Culpitt Rd, Bangor, WI 54614, had his insurance license revoked and was ordered to pay consumer restitution of $298,372.74 and forfeitures of $202,340.78. These actions were taken based on allegations of taking loans from an insurance client, making unsuitable life insurance and annuity sales, and misrepresenting information on insurance applications. This action was taken based on allegations of failing to disclose a criminal conviction on a licensing application, having a criminal conviction that may be substantially related to insurance marketing type conduct, and failing to disclose an administrative action taken by the State of Florida on a licensing application.

Investment Professionals, Inc., 16414 San Pedro Ave., Ste. 300, San Antonio, TX 78232, had its insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Ohio National Life Insurance Company, One Financial Way, Cincinnati, OH 45242, was ordered to pay consumer restitution of $6,626.00. This action was taken based on the sale of an unsuitable insurance product made by one of its appointed insurance intermediaries.
