Madison, Wis. — OCI has taken the following administrative actions. In many of these cases the respondent denied the allegations but consented to the action taken. Any forfeitures paid in these administrative actions are deposited in the Common School Fund which is administered by the Board of Commissioners of Public Lands. The earnings from this fund are distributed to all public K-12 schools in Wisconsin and are used by school libraries to purchase books. Allegations and Actions Against Agents

Chris D. Abeyta, 13570 Meadowgrass Dr., Ste. 100, Colorado Springs, CO 80921, had his application for an insurance license denied. This action was taken based on allegations of having an administrative action taken by the State of Colorado and exhibiting evidence of dishonest practices in the conduct of business.

ZaMiah N. Bell, 1326 Lower Fall Dr., McDonough, GA 30252, had her application for an insurance license denied and her request for a hearing dismissed. These actions were taken based on allegations of engaging in dishonest business practices, making misrepresentations on a licensing application, and failing to appear at a scheduled administrative proceeding.

Harry "Hank" W. Frazee, III, 23679 Calabasas Road, Ste. 309, Calabasas, CA 91302, agreed to the issuance of an insurance license having certain notification and reporting requirements. This action was taken based on allegations of having involvement in a lawsuit containing allegations of fraud and misrepresentation and failing to provide complete licensing application documentation.

Andre Y. Gordon, W163 N5482 Westwind Dr., Menomonee Falls, WI 53051, agreed to pay a forfeiture of $5,000.00 and agreed to the one-year suspension of his insurance license. These actions were taken based on allegations of selling an unsuitable long-term care policy and forging customer signatures on insurance documents.

Ian G. Martin, 760 S. Brevard Ave., Unit 312, Cocoa Beach, FL 32931, agreed to the issuance of an insurance license having certain reporting requirements. This action was taken based on allegations of having involvement in lawsuits containing allegations of fraud, misrepresentation, misappropriation or breach of fiduciary duty, and failing to accurately complete a licensing application.

Charles P. Muhler, 3585 Indian Queen Ln., Philadelphia, PA 19129, was ordered to pay a forfeiture of $500.00 and was ordered to timely and fully respond to all information requests. These actions were taken based on allegations of failing to respond to inquiries from OCI regarding an employment termination for cause.

Mary E. Owen, 1206 Buena Vista Dr., Sun Prairie, WI 53590, agreed to the permanent surrender of her line of authority to sell variable life insurance and variable annuities and agreed not to sell any annuity products for two years. These actions were taken based on allegations of violating the Wisconsin Uniform Securities Law.

Benjamin Pfiffner, 5815 Old Coach Rd., Wausau, WI 54401, had his application for additional insurance authority denied. This action was taken based on allegations of having a previous insurance license revocation and license suspension for violating insurance laws and for selling unsuitable life insurance products.

Gregory W. Rieckhoff, 5325 Riverwood Dr., Savage, MN 55378, had his application for an insurance license denied. This action was taken based on allegations of failing to timely disclose administrative actions taken by the State of Minnesota while previously licensed in Wisconsin.

Eric A. Santucci, AKA Eric Miskowski, 1720 E. Primrose St., Springfield, MO 65804, had his application for an insurance license denied. This action was taken based on allegations of having criminal convictions and an administrative action taken by the State of Wisconsin that may be substantially related to insurance marketing type conduct, and failing to appear at an administrative proceeding as ordered.

Muriel J. Stulen, 9438 S. County Rd. E, Solon Springs, WI 54873, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Eugene T. Walsh III, 424 Sunday Dr., Altoona, WI 54720, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Bill J. Walton, 13570 Meadowgrass Dr., Ste. 100, Colorado Springs, CO 80921, had his application for an insurance license denied. This action was taken based on allegations of failing to disclose an administrative action taken by the Colorado Securities Commissioner, having an administrative action taken by FINRA, and exhibiting evidence of dishonest practices in the conduct of business.

David M. Watters, 1979 Tumblebrook Rd., Neenah, WI 54956, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Emily L. Willoughby, AKA Emily L. Schelvan, P.O. Box 64, Mason, WI 54856, had her application for an insurance license denied. This action was taken based on allegations of failing to disclose criminal convictions on a licensing application and having unpaid civil money judgments.

Allegations and Actions Against Companies

Advice and Life Group, LLC, 36 W. Main St., Ste. 750, Rochester, NY 14614, had its insurance license revoked. This action was taken based on allegations of failing to have a Wisconsin Designated Responsible Licensed Producer (DRLP) and failing to notify OCI of an address change.

Aronson Group, LLC, 1106A Camanche Ave., Clinton, IA 53732, had its insurance license revoked. This action was taken based on allegations of failing to have a Wisconsin Designated Responsible Licensed Producer (DRLP) and failing to notify OCI of an address change.

Ascot Insurance Company, 55 W. 46th St., Fl. 26, New York, NY 10036, was ordered to pay a forfeiture of $5,000.00.  This action was taken based on allegations of violating surplus lines laws and failing to timely file a compulsory surplus calculation.

Bizlinks Insurance Services, Inc., 140 Diamond Creek Pl., Roseville, CA 95747, had its insurance license revoked. This action was taken based on allegations of failing to have a Wisconsin Designated Responsible Licensed Producer (DRLP) and failing to notify OCI of an address change.

Envolve Vision, Inc., 2711 Centerville Rd., Ste. 400, Wilmington, DE 19808, had its insurance license revoked. This action was taken based on allegations of failing to have a Wisconsin Designated Responsible Licensed Producer (DRLP) and failing to notify OCI of an address change.

Golden Harbor Insurance Agency, LLC, 506 W. 5th St., Winona, MN 55987, had its insurance license revoked. This action was taken based on allegations of failing to have a Wisconsin Designated Responsible Licensed Producer (DRLP) and failing to notify OCI of an address change.

Golden Path Property Group, LLC, 6462 N. 66th St., Milwaukee, WI 53223, had its insurance license revoked. This action was taken based on allegations of failing to have a Wisconsin Designated Responsible Licensed Producer (DRLP) and failing to notify OCI of an address change.

HHA, 2200 W. Commercial Blvd., Ste 206, Fort Lauderdale, FL 33309, had its insurance license revoked. This action was taken based on allegations of failing to have a Wisconsin Designated Responsible Licensed Producer (DRLP) and failing to notify OCI of an address change.

Herus Consortium, N19 W24400, Ste. 350, Waukesha, WI 53188, had its insurance license revoked. This action was taken based on allegations of failing to have a Wisconsin Designated Responsible Licensed Producer (DRLP) and failing to notify OCI of an address change.

Homebridge Financial Services, Inc., 194 Wood Ave S., Fl. 9, Iselin, NJ 08830, agreed to apply appropriate measures and agreed to provide supervision to ensure that non-licensed employees do not engage in insurance activities. These actions were taken based on allegations of administrative actions taken by other states based on employees selling products without being appropriately registered or licensed.

Insurance Agency of TD Ameritrade, LLC, 4600 Alliance Gateway Fwy., Fort Worth, TX 76177, had its insurance license revoked. This action was taken based on allegations of failing to have a Wisconsin Designated Responsible Licensed Producer (DRLP) and failing to notify OCI of an address change.

Insurance Canopy, LLC, 3900 Westerre Pkwy., Richmond, VA 23233, had its insurance license revoked. This action was taken based on allegations of failing to have a Wisconsin Designated Responsible Licensed Producer (DRLP) and failing to notify OCI of an address change.

John Alden Life Insurance Company, 501 W. Michigan St., Milwaukee, WI 53203, was ordered to pay a forfeiture of $1,000.00. This action was taken based on allegations of failing to comply with an examination order related to company management and control.

Layne Financial, Inc., 315 Green Ridge Rd., Ste H1, New Castle, PA 16105, had its insurance license revoked. This action was taken based on allegations of failing to have a Wisconsin Designated Responsible Licensed Producer (DRLP) and failing to notify OCI of an address change.

Melior Benefits Agency, LLC, 19337B Katy Fwy., Ste. 281, Houston, TX 77024, had its insurance license revoked. This action was taken based on allegations of failing to have a Wisconsin Designated Responsible Licensed Producer (DRLP) and failing to notify OCI of an address change.

NFS Insurance Agency, LLC, 4640 Admiralty Way, Ste. 200, Marina del Rey, CA 90292, had its insurance license revoked. This action was taken based on allegations of failing to have a Wisconsin Designated Responsible Licensed Producer (DRLP) and failing to notify OCI of an address change.

Opes Financial Solutions, LLC, 401 Isleworth Ln., McKinney, TX 75070, had its insurance license revoked. This action was taken based on allegations of failing to have a Wisconsin Designated Responsible Licensed Producer (DRLP) and failing to notify OCI of an address change.

Paradigm Insurance Group, LLC, 1010 Jorie Blvd., Ste. 248, Oak Brook, IL 60523, had its insurance license revoked. This action was taken based on allegations of failing to have a Wisconsin Designated Responsible Licensed Producer (DRLP) and failing to notify OCI of an address change.

Personalized Brokerage Services, LLC, 4910 Corporate Centre Dr., Ste. 210, Lawrence, KS 66047, had its insurance license revoked. This action was taken based on allegations of failing to have a Wisconsin Designated Responsible Licensed Producer (DRLP) and failing to notify OCI of an address change.

Peter Varghese Agency, Inc., 980 E. Rollins Rd., Round Lake Beach, IL 60073, had its insurance license revoked. This action was taken based on allegations of failing to have a Wisconsin Designated Responsible Licensed Producer (DRLP) and failing to notify OCI of an address change.

Pin Point Marketing, Inc., 318 N. Carson St., Ste. 208, Carson City, NV 89701, had its insurance license revoked. This action was taken based on allegations of failing to have a Wisconsin Designated Responsible Licensed Producer (DRLP) and failing to notify OCI of an address change.

Post Insurance Services, Inc., 2717 W. Bannock St., Boise, ID 83702, had its insurance license revoked. This action was taken based on allegations of failing to have a Wisconsin Designated Responsible Licensed Producer (DRLP) and failing to notify OCI of an address change.

Ramse Insurance Agency, Inc., 133 W. Rand Rd., Rte. 120, Ste. A, Lakemoor, IL 60051, had its insurance license revoked. This action was taken based on allegations of failing to have a Wisconsin Designated Responsible Licensed Producer (DRLP) and failing to notify OCI of an address change.

Security Health Plan of Wisconsin, Inc., 1515 N. Saint Joseph Ave., Marshfield, WI 54449, was ordered to pay a forfeiture of $1,000.00. This action was taken based on allegations of failing to comply with an examination order related to company transactions with affiliates.

Shift Health Solutions, LLC, 2875 NE 191st St., Ste. 500, Aventura, FL 33180, had its insurance license revoked. This action was taken based on allegations of failing to have a Wisconsin Designated Responsible Licensed Producer (DRLP) and failing to notify OCI of an address change.

Timothy Agency, Inc., 3518 55th St. NW, Rochester, MN 55901, had its insurance license revoked. This action was taken based on allegations of failing to have a Wisconsin Designated Responsible Licensed Producer (DRLP) and failing to notify OCI of an address change.

Unified Brokers Alliance, LLC, 2900 S. 70th St., Ste. 300, Lincoln, NE 68506, had its insurance license revoked. This action was taken based on allegations of failing to have a Wisconsin Designated Responsible Licensed Producer (DRLP) and failing to notify OCI of an address change.

Union First Laser Technologies, DBA Union First Insurance, 123 Swiggum Rd, Westby, WI 54667, had its insurance license revoked. This action was taken based on allegations of failing to have a Wisconsin Designated Responsible Licensed Producer (DRLP) and failing to notify OCI of an address change.

Allegations and Actions Against Other Entities

Randy Ziegelbauer, DBA R&N Stainless, c/o Ziegelbauer Fabrication LLC, 188 Scout Rd., Mosinee, WI 54455, was ordered to pay a forfeiture of $1,000.00 and was ordered to cease and desist preparing certificates of insurance. This action was taken based on allegations of preparing, altering or issuing a fraudulent certificate of insurance.
