Madison, Wis. — OCI has taken the following administrative actions. In many of these cases the respondent denied the allegations but consented to the action taken. Any forfeitures paid in these administrative actions are deposited in the Common School Fund which is administered by the Board of Commissioners of Public Lands. The earnings from this fund are distributed to all public K-12 schools in Wisconsin and are used by school libraries to purchase books. Allegations and Actions Against Agents

Sergio Archuleta, 5555 Yellowstone Ave., Trlr. 54, Chubbuck, ID 83202, had his insurance license revoked. This action was taken based on allegations of failing to respond to requests for information; failing to notify OCI of address changes and administrative actions taken by the States of California, Nebraska, South Dakota, Minnesota, Washington, and Virginia; engaging in dishonest business practices; and failing to appear at an administrative proceeding.

James M. Harrington, 8 N. San Pedro St., Ste. 200, San Jose, CA 95110, agreed to pay a forfeiture of $500.00. This action was taken based on allegations of recommending unsuitable life insurance policies.

Emily K. Heim, N7778 567th St., Menomonie, WI 54751, agreed to the revocation of her insurance license and agreed not to reapply for licensure for a period of at least two years. These actions were taken based on allegations of improperly using notes and other reference materials to complete an examination for an insurance license.

Craig W. Henderson, 4607 152nd Ave., Kenosha, WI 53144, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Toni J. Kirkendoll, 1001 Lake Carolyn Pkwy., Apt. 343, Irving, TX 75039, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Brian J. Koenigs, 650 Madison St., Apt. 3, Brillion, WI 54110, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Dirk K. Madison, 3824 N. Bigelow St., Peoria, IL 61614, had his insurance license revoked. This action was taken based on allegations of failing to respond to OCI requests for information, failing to notify OCI of address changes, failing to file a timely answer, failing to appear at an administrative proceeding, and engaging in dishonest business practices.

Matthew A. Malone, 6222 Tranquil River Ln., Wausau, WI 54401, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Edgar Jose Martinez, 36 Grand Canyon Dr., Madison, WI 53705, agreed to the permanent surrender of his insurance license and agreed to pay a forfeiture of $5,000.00 upon any future Wisconsin licensing application. These actions were taken based on allegations of obtaining a personal loan from an insurance client.

Courtney N. McAdoo, 6434 Ziegler Ln., Charlotte, NC 28269, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

William J. Schnepp, N63 W23309 Main St., Apt. 103, Sussex, WI 53089, agreed to the permanent surrender of his insurance license and agreed to pay a forfeiture of $20,000.00 upon any future Wisconsin licensing application. These actions were taken based on allegations of engaging in personal financial transactions with an insurance client, committing theft from an insurance client, providing false and misleading communications to an insurance client and insurance company, and engaging in other dishonest business practices.

Aaron M. Stolp, 11124 14th St. NE, Hanover, MN 55341, agreed to the denial of his licensing application, agreed to withdraw his hearing request, and agreed to submit complete and truthful licensing applications on or after January 1, 2020 only if he is not named in any new civil, administrative, or criminal proceedings. These actions were taken based on allegations of failing to disclose criminal convictions on licensing applications and having civil and criminal convictions that may be substantially related to insurance marketing type conduct.

Justin Tudhope, 1606 Gateway Blvd., Apt. 12, Beloit, WI 53511, had his insurance license revoked. This action was taken based on allegations of failing to pay a licensing fee.

Jason J. Webb, 1303 Pulaski Rd., Buffalo, MN 55313, was ordered to pay a forfeiture of $1,500.00 and was ordered to timely and fully report in the future all matters required to be disclosed. These actions were taken based on allegations of failing to timely report administrative actions taken by the State of Minnesota and failing to disclose them on a licensing renewal application.

Douglas C. Woodard, Jr., S71 W16128 Susan Ct., Muskego, WI 53150, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Greg H. Zastrow, W5910 Sumac Rd., Plymouth, WI 53073, agreed to pay a forfeiture of $2,000.00 and agreed to pay consumer restitution of $9,232.00. These actions were taken based on allegations of failing to comply with a previous administrative order, failing to disclose an administrative action taken by the State of Wisconsin on a licensing renewal application, and recommending unsuitable annuity transactions.
