Madison, Wis. — OCI has taken the following administrative actions. In many of these cases the respondent denied the allegations but consented to the action taken. Any forfeitures paid in these administrative actions are deposited in the Common School Fund which is administered by the Board of Commissioners of Public Lands. The earnings from this fund are distributed to all public K-12 schools in Wisconsin and are used by school libraries to purchase books. Allegations and Actions Against Agents

Thomas E. Behling, W5673 Roberts Rd., Tomahawk, WI 54487, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Jessica Bystry, 1700 30th Ave., Apt. 11, Monroe, WI 53566, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Yolanda R. Cain, 4657 W. County Line Rd., Brown Deer, WI 53223, had her application for an insurance license denied and her request for a hearing on the denial decision dismissed. These actions were taken based on allegations of failing to fully disclose administrative actions taken by the State of Wisconsin on a licensing application, failing to accurately complete an application form, having unpaid civil money judgments, violating the terms of a previous order of the Commissioner, failing to respond to OCI requests for information, having an unpaid lien from the Wisconsin Department of Children and Families, and failing to appear at a scheduled administrative proceeding.

Mark R. Devereux, 408 Chestnut Dr., New Richmond, WI 54017, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Theodore R. Frey, 5916 Hilldale Dr., Hartford, WI 53027, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Bernabe Gonzalez, 451 N. Moreland Blvd., Waukesha, WI 53188, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Precious J. Hill, 2823A W. McKinley Blvd., Milwaukee, WI 53208, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Jamie Jackson, 1734 S. 2nd St., Upper, Milwaukee, WI 53204, had his application for an insurance license denied. This action was taken based on allegations of failing to disclose a criminal conviction on a licensing application and engaging in dishonest practices in the conduct of business.

Weston D. Mock, 3606 N. 163rd Plaza, Omaha, NE 68116, agreed to the surrender of his insurance license and agreed to pay a forfeiture of $1,000.00 upon any license reapplication. These actions were taken based on allegations of misrepresenting information submitted with insurance licensing applications.

Sara E. Morris, 2009 29th St. S., La Crosse, WI 54601, had her application for an insurance license denied. This action was taken based on allegations of having criminal convictions that may be substantially related to insurance marketing type conduct and having unpaid civil money judgments.

Benjamin T. Pechacek, W310 S2580 Cregennan Bae, Wales, WI 53183, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Anthony F. Raad, a/k/a Ramzi F. Raad, 8458 Sunset Rose Dr., Corona, CA 92883, had his application for an insurance license denied. This action was taken based on allegations of having administrative actions taken by the States of California and Wisconsin; violating insurance laws; and using fraudulent, coercive, or dishonest practices, or demonstrating incompetence, untrustworthiness, or financial irresponsibility in the conduct of business.

Sapan N. Shah, 5325 Reserve Cir., Cincinnati, OH 45230, had his application for an insurance license denied. This action was taken based on allegations to respond promptly to inquiries from OCI and failing to disclose administrative actions taken by the States of Wisconsin, Ohio, and New York on a licensing application.

Maria G. Tello-Valdez, 600 W. Wisconsin St., Sparta, WI 54656, had her application for an insurance license denied for 60 days. This action was taken based on allegations of failing to accurately complete a licensing application.

Matthew A. Teresi, Sr., 3612 Harmony Ln., South Milwaukee, WI 53172, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Christiana S. White, 4722 N. 85th Ct., Milwaukee, WI 53225, had her application for an insurance license denied. This action was taken based on allegations of having a criminal conviction that may be substantially related to insurance marketing type conduct, providing misleading information on a licensing application, and having unpaid civil money judgments and tax liens.

Allegations and Actions Against Companies

Group Dental Service, Inc., 15400 Calhoun Dr., Ste 300, Rockville, MD 20850, had its insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
