Madison, Wis. — OCI has taken the following administrative actions. In many of these cases the respondent denied the allegations but consented to the action taken. Any forfeitures paid in these administrative actions are deposited in the Common School Fund which is administered by the Board of Commissioners of Public Lands. The earnings from this fund are distributed to all public K-12 schools in Wisconsin and are used by school libraries to purchase books. Allegations and Actions Against Agents

Jabari J. Courtney, 6820 Evanton Loch Rd., Charlotte, NC 28278, was ordered to pay a forfeiture of $1,000.00. This action was taken based on allegations of failing to respond promptly to inquiries from OCI and failing to timely notify OCI of an address change.

Darren Gooding, 154 Berger St., Apt. 8, Green Bay, WI 54302, agreed to pay a forfeiture of $1,000.00. This action was taken based on allegations of failing to timely report an administrative action taken by the State of California and failing to timely disclose a criminal prosecution to OCI, as well as failing to disclose the actions on a licensing renewal application.

Jeremiah M. Lozano, 814 Jordan Rd., De Pere, WI 54115, agreed to the issuance of a two-year restricted insurance license with certain reporting requirements. This action was taken based on allegations of failing to disclose criminal convictions on a licensing application and having criminal convictions that may be substantially related to insurance marketing type conduct.

John J. Mueller, 601 S. Central Ave., Marshfield, WI 54449, had his application for an insurance license denied for 30 days. This action was taken based on allegations of failing to disclose criminal convictions on a licensing application.

Christopher L. Porter, 13514 Grissom St., Omaha, NE 68138, agreed to the revocation of his insurance license, agreed not to reapply for licensure for at least two years, and agreed to pay a forfeiture of $1,000.00 upon reapplication. These actions were taken based on allegations of having an appointment with an insurer terminated for submitting fraudulent life insurance applications and failing to timely disclose administrative actions taken by the States of Idaho, Kansas, Washington, Indiana, and Nebraska.

Tanner Rego, 2371 Doris Rd., Reedsburg, WI 53959, agreed to the issuance of a two-year restricted insurance license with certain employment and reporting requirements. These actions were taken based on allegations of having criminal convictions that may be substantially related to insurance marketing type activities and having unpaid civil money judgments.

Joel J. Spelman, 1427 W. College Ave., Appleton, WI 54914, had his application for a federal crime waiver denied. This action was taken based on allegations of failing to complete the waiver application.

Xao K. Vang, 1749 Lilac Ln., Green Bay, WI 54302, had his application for an insurance license denied. This action was taken based on allegations of failing to disclose administrative actions taken by the State of Wisconsin, failing to disclose an insurance company lawsuit, and failing to disclose an employment termination for cause on a licensing application.

Lawrence Ventresca, 4839 N. Canfield, Norridge, IL 60706, agreed to pay a forfeiture of $1,750.00. This action was taken based on allegations of failing to timely disclose administrative actions taken by the State of Louisiana and FINRA, and failing to disclose those actions on licensing applications.

Rui Wang, 4917 N. Shoreland Ave., Whitefish Bay, WI 53217, agreed to the issuance of a temporary insurance license. This action was taken based on allegations of failing to provide evidence of authorization to work in the United States beyond the standard licensing period.

Stephanie Whalen, 5688 Clouds Mill Dr., Alexandria, VA 22310, agreed to the 60-day denial of her licensing application, agreed to future disclosure requirements, and agreed to abide by all Wisconsin insurance laws and regulations. These actions were taken based on allegations of failing to disclose a criminal conviction that may be substantially related to insurance marketing type conduct on a licensing application.

Allegations and Actions Against Companies

ManhattanLife Assurance Company of America, 10777 N.W. Freeway, Houston, TX 77092, was ordered to pay a forfeiture of $1,000.00 and was ordered to provide information to OCI. These actions were taken based on allegations of failing to respond promptly to inquiries from OCI.

Smart Motors, Inc., 5901 Odana Rd, Madison, WI 53719, was ordered to pay an annual fee and was ordered to provide required information to OCI. These actions were taken based on allegations of failing to timely pay an annual licensing fee and failing to provide required financial and other information.

Zacho Sports Center, Inc., 2449 S. Prairie View Rd., Chippewa Falls, WI 54729, was ordered to pay an annual fee and was ordered to provide required information to OCI. These actions were taken based on allegations of failing to timely pay an annual licensing fee and failing to provide required financial and other information.
