Madison, WI—OCI has taken the following administrative actions. In many of these cases the respondent denied the allegations but consented to the action taken. Any forfeitures paid in these administrative actions are deposited in the Common School Fund which is administered by the Board of Commissioners of Public Lands. The earnings from this fund are distributed to all public K-12 schools in Wisconsin and are used by school libraries to purchase books. Allegations and Actions Against Agents

Rachel A. Allen, aka Rachel A. Thome, 1016 11th Ave., Bloomer, WI 54724, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Sharon D. Bell, 803 S. Lincoln Ave., Apt. 6, Beaver Dam, WI 53916, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

John S. Bostwick, 333 S. Stephenson Ave., Iron Mountain, MI 49801, agreed to pay a forfeiture of $500.00. This action was taken based on allegations of failing to disclose an annuity replacement transaction.

Alli M. Griffith, 2700 Shannon Ln., Mound, MN 55364, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Edward Klug, 124 W. Marquette St., Appleton, WI 54911, had his application for an insurance license denied. This action was taken based on allegations of making material misrepresentations and failing to disclose administrative actions on a licensing application, applying for unauthorized insurance authority, and failing to timely disclose an initial pretrial hearing while previously licensed.

Bill G. Marks, 2637 N. 61st St., Milwaukee, WI 53213, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Natalie D. Miller, fka Dustin P. Miller, 969 N. Gammon Rd. Apt. 8, Madison, WI 53717, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Jacob C. Reidenbach, 669 Walsh Rd., Hudson, WI 54016, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Herbert Russell, 804 E. 26th St., Norfolk, VA 23504, had his application for an insurance license denied. This action was taken based on allegations of having administrative actions taken by the states of Maryland and California, and failing to respond to inquiries from OCI.

Katherine J. Smith, 365 Hoover St., Sun Prairie, WI 53590, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Kenya Tucker, 5029A N. 28th St., Milwaukee, WI 53209, had her insurance license revoked. This action was taken based on allegations of failing to comply with the terms of a previous Stipulation and Order.

Kenya Tucker, 5029A N. 28th St., Milwaukee, WI 53209, had her application for additional lines of insurance authority denied. This action was taken based on allegations of failing to disclose an administrative action taken by the state of Wisconsin on a licensing application, failing to comply with the terms of a probationary insurance license, and failing to timely disclose a lawsuit and address change.

Nanette N. Vorath, 4122 N. 70th St., Milwaukee, WI 53216, was ordered to apply for and be granted a federal crime waiver before adding additional licensing authority. This action was taken based on allegations of failing to obtain a required licensing qualification.

Jennifer L. Weisfeld, 204 N. Union Rd., Manitowoc, WI 54220, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Ralph E. Wentworth, 12 Stuart Dr., Bloomfield, CT 06002, agreed to the issuance of a two-year probationary insurance license with certain reporting requirements. This action was taken based on allegations of having administrative actions taken by the states of Kansas, New York, Massachusetts, Alaska, and Hawaii.

Lance E. Writt, 314 N. Van Buren St., Green Bay, WI 54301, had his application for an insurance license denied. This action was taken based on allegations of having criminal convictions that may be substantially related to insurance marketing type conduct; providing incorrect information on a licensing application; being involved in lawsuits or arbitrations containing allegations of fraud, misrepresentation, misappropriation, or breach of fiduciary duty; and failing to obtain a federal crime waiver.

Lance E. Writt, 314 N. Van Buren St., Green Bay, WI 54301, had his application for a federal crime waiver denied. This action was taken based on allegations of having felony convictions involving dishonesty or breach of trust.

Allegations and Actions Against Companies

ESP Financial Strategies, LLC, 512 Spring St., Pewaukee, WI 53072, had its insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
