HMO Data Collection

The HMO Data Collection report is required to be filed with the Office of the Commissioner of Insurance. Insurers offering HMO plans in Wisconsin must annually submit data elements specified by the Commissioner per s. Ins 9.40 (5), Wis. Adm. Code.

OCI has created an online interface for insurance companies to interact with the Wisconsin Market Regulatory Reports and Forms application. Each insurer's Wisconsin Market Regulatory Reports and Forms Security Administrator/Forms Administrator will assign the HMO Data Collection report to staff for completion. The forms will be completed utilizing the online Wisconsin Market Regulatory Reports and Forms application. This report will be available May 2017.
