The purpose of this form is to allow companies to calculate the compulsory surplus for companies that are not licensed in Wisconsin or to perform a calculation that will not be filed with OCI.
Licensed companies will continue to be required to file the form electronically via the internet. Failure to do so may result in administrative action. Companies that are applying for licensure may print out the calculation for submission with their application materials.
