A care management organization as defined in s. 648.01, Wis. Stat., must obtain a permit as a care management organization. Please complete and return the application for a permit for a care management organization (link provided below). Any questions about the application should be directed to ocicompanylicensing@wisconsin.gov.
No application will be addressed until all items on the application form have been completed.
