Information on the Acquisition of control of My Choice Wisconsin Health Plan Inc., and My Choice Wisconsin Inc. by My Choice Foundation Holdings Inc.

​Effective April 12, 2022, this Form A filing was withdrawn.​

On October 25, 2021, My Choice Foundations Holdings Inc. (“MCFH", the “Applicant") submitted a Form A filing regarding the proposed acquisition of My Choice Wisconsin Health Plan Inc. (“MCWHP"), and My Choice Wisconsin Inc. (“MCW"). Subject to MCFH's recognition by the Internal Revenue Service as an IRC Section 501(c)(3) supporting organization, MCW approved on October 15, 2021, Amended and Restated Articles of Incorporation and Amended and Restated Bylaws. This reorganization will result in MCWHP being wholly owned by MCW and MCW being wholly owned by MCFH. As this is a reorganization, no consideration will be exchanged.
