Subject: Changes in Rate and Form Filing Procedures


​Wisconsin is now accepting the NAIC Uniform Transmittal Documents and the Uniform Product Coding Matrices (UPCM). You can access these forms at the NAIC website: https://content.naic.org/​​. Click on the links for the transmittal documents. There are two documents, one for property and casualty insurance and one for life and health insurance. The documents are used for both rate and form filings. Property and casualty insurers, please continue to submit separate filings for rate and form filings.

Click on the links for Uniform Product Coding Matrices to select the codes you will use when submitting a filing.

INSURERS ARE ENCOURAGED TO START USING THE NEW CODING SYSTEM IMMEDIATELY.

CERTIFICATE OF COMPLIANCE (see s. Ins 6.05, Wis. Adm. Code, Appendix A)
A completed certificate of compliance, signed and dated must be submitted for each company in the filing.

INFORMATION ABOUT CERTIFYING READABILITY (see s. Ins 6.07 (6), Wis. Adm. Code, for instructions). Some forms require a certificate of readability. See the rule for details.
