Madison, Wis. — The Office of the Commissioner of Insurance (OCI) is responsible for administering and enforcing the insurance laws of Wisconsin. This includes monitoring the financial and marketing practices of individuals and companies. Each month, OCI publishes the administrative actions it has taken against these entities. The actions are issued by OCI when licensing applicants, or licensed individuals or companies have violated Wisconsin insurance statutes or regulations. In some cases, the individual or company denied the stated allegations but consented to the action taken. OCI strives to ensure fair and honest business practices to protect Wisconsin insurance consumers through this type of transparency. Any forfeitures paid are deposited in Wisconsin's Common School Fund administered by the Board of Commissioners of Public Lands. Earnings from the Fund are distributed to all public K-12 schools in the state and are used by school libraries to purchase materials such as books, newspapers, and computers.

Allegations and Actions Against Agents

Damion L. Ashford, 3246 N. 11th St., Milwaukee, WI 53206, had his application for an insurance license denied.  This action was taken based on allegations of having a criminal conviction that may be substantially related to the circumstances of holding an insurance license.

Blain Clemons, 3480 Golfview Dr., Unit 1209, Eagan, MN 55123, had his insurance license revoked.  This action was taken based on allegations of owing delinquent Wisconsin taxes.

Anna Collins, 3007 NE 73rd Cir., Vancouver, WA 98665, agreed to the suspension of their Wisconsin insurance license. This action was taken based on allegations of switching consumers' health insurance plans without their knowledge or consent.

Peter M. Cymbalak, 8383 Greenway Blvd., Ste. 210, Middleton, WI 53562, had his insurance license revoked.  This action was taken based on allegations of owing delinquent Wisconsin taxes.

Michael Diblasi, 241 3rd St., Hanover, PA 17331, was ordered to pay a forfeiture of $500.00.  This action was taken based on allegations of failing to report an administrative action taken by another state.

Cameron Gaines, 1717 Naperville Rd., Ste. 300, Naperville, IL 60563, had his insurance license revoked.  This action was taken based on allegations of owing delinquent Wisconsin taxes.

Marcus Grant, 62 NW 161st St., Miami, FL 33169, was ordered to pay a forfeiture of $500.00.  This action was taken based on allegations of failing to report an administrative action taken by another state.

Michael G. Lecher, N4905 Hillwind Rd., Plymouth, WI 53073, had his insurance license denied for 30 days. This action was taken based on allegations of having a criminal conviction which may be substantially related to the circumstances of holding an insurance license.

Steven V. Lisiewicz, 439 Mooresfield, Elgin, IL 60124, agreed to the suspension of his Wisconsin insurance license. This action was taken based on allegations of untrustworthiness.

Stephanie Moody, 628 335th Ave., Frederic, WI 54837-4854, had her insurance license revoked.  This action was taken based on allegations of owing delinquent Wisconsin taxes.

Christina J. Muha, 334 W. 806 N., Valparaiso, IN 46835, agreed to surrender her public adjuster registration.  This action was taken based on allegations of not maintaining a public adjuster bond.

Jared J. Nagel, 3391 Royal Oak Dr., Plover, WI 54467-3785, was ordered to pay a forfeiture of $1,500.00.  This action was taken based on allegations of failing to timely report a criminal conviction and failing to timely disclose criminal charges.

Gloria Neal, 3303 Toledo Dr., Apt. A, Killeen, TX 76542, had her insurance license revoked.  This action was taken based on allegations of owing delinquent Wisconsin taxes.

Adrian Raventos, 4425 SW 160th Ave., Apt. 109, Miramar, FL 33027-5741, was issued a forfeiture of $500.00. This action was taken based on allegations of failing to report an administrative action taken by another state.

Amar P. Reddy, 601 E. Geneva St., Elkhorn, WI 53121, had his insurance license revoked.  This action was taken based on allegations of owing delinquent Wisconsin taxes.

Patricia Smith, 1551 Creekwood Dr., Troy, OH 45373, agreed to the permanent revocation of their Wisconsin insurance license. This action was taken based on allegations of engaging in fraudulent conduct in the solicitation and sale of life insurance policies.

Christopher L. Stanley, 28738 SW 134th Ct., Homestead, FL 33033, was ordered to pay a forfeiture of $1,000.00. This action was taken based on allegations of failing to timely report an administrative action taken by another state.

Seth Thephavong, 1570 Louise St., Green Bay, WI 54302-2438, had his insurance license revoked.  This action was taken based on allegations of owing delinquent Wisconsin taxes.

Michael Usbelger, 380 Lafayette Rd., Unit 334, Seabrook, NH 03874-4551, agreed to the permanent revocation of their Wisconsin insurance license. This action was taken based on allegations of engaging in fraudulent conduct in the solicitation and sale of life insurance policies.

Jay R. Weiser, PO Box 7, Earlville, IL 60518-0007, had his application for an insurance license denied. This action was taken based on allegations of having a permanent bar from FINRA and failing to timely disclose information regarding regulatory actions.

Allegations and Actions Against Companies

Allied Administrators Inc., PO Box 70930, Oakland, CA 94612, was ordered to pay a forfeiture of $500.00.  This action was taken based on allegations of failing to report an administrative action taken by another state.

BMT Insurance Advisors Inc., 100 Granite Dr., Ste. 205, Media, PA 19063, was ordered to pay a forfeiture of $1,000.00.  This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.

Ballazhi and Associates LLC, 624 W. Happfield Dr., Arlington Heights, IL 60004, was ordered to pay a forfeiture of $500.00.  This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.

Universal Fidelity Life Insurance Company, 13931 Quail Pointe Dr., Oklahoma City, OK 73134-1039, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to timely report an administrative action taken by another state.​
