Insurers wishing to offer dental plans in Wisconsin must comply with all policy form and rate filing requirements. The Dental Plans Policy Form and Rate Filing Checklist at the end of this page is provided for guidance.
- Forms are to be filed with the Office of the Commissioner of Insurance (OCI) at least 30 days prior to use (per s. 631.20, Wis. Stat.).
- Rates are to be filed with OCI within 30 days after they become effective (per s. 625.13, Wis. Stat.).
Exchange-Certified Dental Plans
Dental plans certified by the federal exchange must include pediatric dental essential health benefits. These stand-alone dental plans (SADPs) must also meet Wisconsin requirements and file their rates and forms with OCI.
SADPs seeking exchange certification for plan year 2025 (whether offered on- or off-exchange) must submit all relevant policy forms and rates to OCI (or have a submission date in SERFF) no later than June 12, 2024.
