If your company was the non-survivor in a merger or conversion transaction, filing of your Schedule of Fees and Fire Dues form and payment should be made by paper form and check sent directly to this office. Non-survivor company FEINs will not be recognized by our electronic payment system, nor by the US Bank drawer.
