Information on the Merger of Anthem, Inc. and Wellpoint Health Networks, Inc., and the Acquisition of Control of Crossroads Acquisition Corp. and its Wisconsin Domestic Insurance Subsidiaries
On December 17, 2003, Anthem, Inc., filed a statement regarding the acquisition of control of or merger with a domestic insurer pursuant to s. 611.72, Wis. Stat. The Wisconsin domestic insurers affected by this transaction are Blue Cross & Blue Shield United of Wisconsin, Compcare Health Services Insurance Corporation, Unity Health Plans Insurance Corporation, United Heartland Life Insurance Company, United Wisconsin Insurance Company, and Valley Health Plan, Inc.
The plan of acquisition, along with all pertinent correspondence and statutorily required information, will be placed in this section of the OCI's Web site. Forms and other documents filed by Anthem, Inc. and Wellpoint Health Networks, Inc. with the U.S. Securities and Exchange Commission (hereinafter also, SEC) pertaining to this transaction may be accessed on that federal agency's website at
