The Office of the Commissioner of Insurance (OCI) frequently provides speakers on a variety of insurance issues. If you would like someone from OCI as a speaker, please complete the form (using the link below) in its entirety so that OCI staff can check the availability of speakers and resources for the issues you are requesting. Allow two to three weeks for processing and please note that later changes to the original request may affect speaker availability.

To complete the form, simply click on the link below and fill in the data fields on the form. Push the "Submit Data & Print" button and the information will be submitted to OCI and you will have the opportunity to print a copy. It is recommended that you print out a copy for your records.
