Subject: Changes in Form Filing Procedures


​Wisconsin has new requirements for submitting form filings, effective immediately. Companies must submit a separate insurance form listing for each Product Category (formerly called coverage class). We have also revised some of the Product Codes (formerly called coverage codes) that go with each Product Category. The changes affect what you enter into columns #8 and #9 of the Insurance Form Listing (OCI 26-015). We will revise the insurance form listing, but in the interim, insurers will need to list the Product Category next to column #8, leave column #8 blank and insert the 3-character Product Codes for the products in column #9. The Type of Filing codes have not changed.

A summary of all valid Product Categories and Product Codes is attached. ONLY ONE PRODUCT CATEGORY IS PERMITTED PER FILING. INSURERS WILL NEED TO SUBMIT SEPARATE FILINGS FOR EACH PRODUCT CATEGORY. INCORRECT FILINGS WILL BE CONSIDERED INCOMPLETE AND WILL BE RETURNED WITHOUT REVIEW.

INSURERS SHOULD START TO USE THE NEW CODING SYSTEM IMMEDIATELY.

TYPE OF FILING
The codes for Type of Filing (column #10) have not changed.

CERTIFICATE OF COMPLIANCE (see Ins. 6.05, Wis. Adm. Code, Appendix A)
A completed certificate of compliance, signed and dated must be submitted for each company in the filing.
