Wisconsin is now accepting electronic appointments and terminations. Log on to www.nipr.com to obtain information on vendors authorized by NIPR or www.sircon.com/wisconsin for information on services provided by Sircon Corporation.

Please begin using the new forms as soon as possible. OCI will discontinue acceptance of all previous forms after January 1, 2002.
